Welcome to the Heart Failure Academy

The Heart Failure Academy, funded by NHS England, is a collaboration across all three Yorkshire and Humber Integrated Care Systems (ICBs) with dedicated support from Leeds Teaching Hospitals NHS Trust, Health Innovation Yorkshire & Humber and Pumping Marvellous.

Most people in the UK are diagnosed with heart failure in hospital. A recent study showed 80% of people in the UK were first diagnosed with heart failure during a hospital admission even though 40% had presented with symptoms in primary care within the last five years. 1 Not only is this traumatic for the individual, and places a high burden on secondary care, evidence also shows that those diagnosed with heart failure in an emergency admission will have significantly worse outcomes than those diagnosed in primary care. 2

The aim of the Heart Failure Academy is to support healthcare professionals in recognising the signs and symptoms of heart failure, enabling more timely specialist assessment and diagnosis. This ensures that patients receive the right treatment at the right time, leading to better outcomes for people living with heart failure in Yorkshire and the Humber.

Our innovative map ‘Navigating the heart failure journey’ offers short learning bursts showing how to support someone living with heart failure to live well as their disease progresses.

Each position on the map provides key learning the whole multidisciplinary team can access, enabling the whole team to recognise new or worsening heart failure symptoms, ensuring people with these symptoms are screened and escalated to specialist review promptly, supporting better outcomes in Yorkshire and the Humber.
