‘Navigating the heart failure journey in primary care: a personalised approach to living well’
The Heart Failure Academy and the Pumping Marvellous Foundation are delighted to launch their new collaboration, co-designed by people living with heart failure, generalist and heart failure specialists from across the multidisciplinary team in all three Yorkshire and Humber Integrated Care Boards.
This resource equips those who are new to supporting people living with heart failure with practical tools and guidance. It enables healthcare professionals to empower patients and their families to better understand their condition, recognise deterioration early, and actively participate in their care.
Aligned with the Cardiovascular Disease Modern Service Framework, the book guides clinicians through every stage of the Navigating the Heart Failure Journey map, from symptom recognition and NT-proBNP testing to specialist assessment, diagnosis, treatment initiation, cardiac rehabilitation, long-term management, and advance care planning.
Throughout the heart failure journey described, the resource emphasises the importance of managing associated cardiovascular, kidney, and metabolic conditions, offering a holistic and personalised approach to care.

Each practice in Yorkshire and the Humber will receive five copies of the booklet between October and November 2026.
